Transcript Request Forms:

• Waiver of Rights (to release transcripts to post-secondary institutions)
• Transcript request form (seniors only; scholarships only, college transcript requests go through Naviance)
• Transcript request form for non-seniors
• Transcript request form for alumni

Permit Forms:

• Work Permit Forms are available here. The student prints it out and gives it to their employer to complete. The student and the employer keep a copy.
• MVA Learner’s Permit form available here. The student prints it out, completes their portion, and brings it to Ms. Ichrist at the attendance window.
