I need to complete the Attendance form from the MVA for my student's learner's permit

Maryland State Law requires all applicants for a learner's permit, under the age of 16, to demonstrate acceptable school attendance in the prior school semester. Please use this form to verify and certify School Attendance for a student.  To verify school attendance, School Official or Home School Coordinator must complete Sections B and D of this form.
