Fall Sports Sign-Up Night, Monday, August 6th, 2018

4:00-5:30PM in the MTH Cafeteria

All Students interested in trying out for a Fall Sport at Mt. Hebron High School are urged to attend our Fall Sports Sign-up Night on Monday, August 6th, 2018, from 4:00-5:30PM in the MTH Cafeteria. Please bring with you the following Eligibility Paperwork to our Sports Sign-Up Night: (Bold indicates forms can be down loaded at mhhs.hcpss.org): 

• Pre-Participation Physical Examination (dated on/after April 1, 2018);
• Sudden Cardiac Arrest Acknowledgement Form (Parent/Guardian AND Student Athlete Must Sign);
• Parent Permission Form;
• Photo copy of their Birth Certificate (only if this is their first time participating in a sport at MTH);
• Bring a copy of their 4th Quarter Report Card. Students MUST meet the HCPSS Academic Eligibility Requirements of no more than ONE “E” grade, AND a Minimum GPA of 2.0 or better on their 4th Quarter report Card in order to be Academically Eligible to participate in try-outs.                                        ***INCOMING 9th GRADE STUDENTS ARE EXEMPT FROM THE HCPSS ACADEMIC ELIGIBILITY REQUIREMENTS for the FALL SEASON, ONLY); 
• Residency Verification (in the form of a BGE Bill, Water Bill or Land Line phone bill - required each of the three athletic seasons);
• Concussion Testing is Optional. MTH will offer concussion testing on Monday, August 6th from 3:30-6:00PM in the Media Center. A parent permission slip is required an may be obtained at hcpss.org.
• Fall Sports try-outs begin Wednesday, August 8th, 2018. Students MUST meet ALL the Eligibility Participation requirements above and hand in all necessary paperwork ON OR BEFORE August 8th, in order to participate in Fall Sports try-outs.

Students MUST be in attendance for Try-outs beginning August 8th thru the try-out period in order to be considered for the team. Parents/Guardians, please schedule your summer vacations with this start date in mind.
